Portage DD Request for Proposal (RFP) - Short Term Respite

The Portage County Board of Developmental Disabilities (Portage DD) is requesting written proposals from qualified agency Medicaid waiver providers or ICF providers to operate, a therapeutic respite option for families with children who have developmental disabilities. This would-be short-term respite that would range anywhere from a few hours to a week. This would be for children aged 6 to 21 that are eligible for services at Portage DD. The objective would be to have a 4-bed placement for youth. It is the intent of this program to provide a local respite option to reduce the need for out-of-home placements, allow families needed respite, assist with kids that have significant behavioral and or medical issues, and give families support to strengthen the dynamics in the home
