Supporting Active Community Engagement to Fulfill Dreams and Goals

In this class, participants will demonstrate an understanding of the concept of Person-Centered Planning as it relates to exploring and achieving goals with people who are served by the support team. Class participants will discuss how to use Person-Centered Planning to identify and facilitate actions to help individuals who are supported live happy and fulfilled lives. This class will work together to identify steps to build a collaborative team to support the individual’s goals.
