DSP Award Recipient - Leaty Evans

The Portage County Board of Developmental Disabilities is proud to recognize Leaty Evans as a Portage DD Direct Support Professional (DSP) Award Recipient.  Leaty works for Ultimate Beginnings and was nominated by Allison.

Here is what Allison had to say about Leaty:  Leaty goes above and beyond in her job duties to make sure I’m happy and healthy. She picks up a lot of the call offs for my house. She is very nice.   Shelmin Evans, President/COO of Ultimate Beginnings had this to add:  Leaty has been a part of our team 2022. She’s reliable and engages with the individuals she works with. She’s willing to step in and work wherever is needed and shows respect and patience to the individuals she serves. Leaty is someone we’ve been able to count on and is truly a model DSP. 

This award is an opportunity to nominate someone who has gone above and beyond to improve the lives of individuals with developmental disabilities while also promoting independence and inclusion.

Individual award recipients will receive public recognition from Portage DD and be promoted on our social media channels. Award recipients will also later be considered for Direct Support Professional of the Year.

All nominations will be kept on file for review and consideration for a period of 12 months after initial submission.
